Salem-Keizer School District 24J is required by state law to keep a record of each student’s immunizations. Parents who are enrolling a student in school must complete a Certificate of Immunization Status form with their child's record of immunizations.
Kindergarten students are not allowed to attend school until they have a minimum of one dose each of required vaccines.
Students may be exempted from receiving certain immunizations for medical conditions or non-medical reasons. A new process for claiming a non-medical exemption (formerly called religious exemption) will start March 1, 2014.
After this date, parents choosing a non-medical exemption to immunization requirements will be required to submit to the school or childcare a document showing either:
- a signature from a health care practitioner verifying discussion of the risks and benefits of immunization, OR
- a certificate of completion of an interactive online educational video about the risks and benefits of immunization.
Where Can You Get Immunizations?
Immunizations are available at your student's doctor's office, at many pharmacies, or at county health departments. Call for information or to schedule an appointment.
